Case Management

intensive & Individualized Case Management

All youth entering the shelter for services will be assigned a Case Manager. The Case Manager reviews and completes the intake paperwork, assists in developing a Service Plan with the youth and their parent/guardian to be implemented throughout the youth’s stay at shelter. The Case Manager will also provide individual support, family mediation, and referrals to address needs for the youth and their family during the youth's time at the shelter. When family reunification is not a workable goal, the Case Manager works to help the youth identify alternatives and develop an agreeable transition plan.

The Hearts With A Mission Case Manager ensures that youth are registered and attending school, help set-up transportation to and from school, as well as facilitate transportation to and from any medical/mental health appointments. In addition, the Case Manager also meets with youth weekly to address any challenges and behavior issues that may be present.

​The Case Manager works with each youth to determine which external services are needed and makes the connections for those services if they are not provided on-site. Examples include educational and job readiness programs, medical care, public assistance, and substance abuse treatment needs. The Case Manager works intensively with youth to exit to stable housing, whether with family or to an alternative living situation. At Hearts With A Mission, family reunification is always a priority.
